Welcome back. In our last waypoint, we established why human factors are critical to aviation safety and why we cannot rely on technology alone to prevent accidents. Now, we will look at how we analyze the human operating within a complex aviation system. To do this, we rely on two foundational frameworks: the SHEL model and the concept of the “Dirty Dozen”.

The SHEL model was designed to illustrate the continuous interaction between the human being and all other elements of the aviation system. SHEL is an acronym that stands for Software, Hardware, Environment, and Liveware. In this model, you—the aviation professional—are the central ‘Liveware’ component. You do not operate in a vacuum; you constantly interface with the other three elements to accomplish your daily tasks.

Let’s break down these specific interfaces. ‘Software’ refers to the non-physical aspects of the system, such as the rules, procedures, maintenance data, and computer systems you use. A Liveware-Software mismatch occurs when a Part-CAMO planner struggles with an ambiguous Airworthiness Directive, or a Part-145 technician tries to follow a poorly written maintenance manual. ‘Hardware’ represents the physical aircraft, tooling, and test equipment. A Liveware-Hardware mismatch happens if equipment is poorly designed, uncalibrated, or a specific tool is completely unavailable.

‘Environment’ encompasses the physical workspace, such as hangar lighting, temperature, and noise. It also includes the social and organizational environment, like peer pressure or intense commercial demands. Finally, the Liveware-Liveware interface represents how you interact with other people: your colleagues, supervisors, and flight crews. The vast majority of maintenance errors occur at these interfaces when there is a mismatch or friction between the human and the system.

To make these interface mismatches easier to identify on the hangar floor and in the planning office, Gordon Dupont developed the “Dirty Dozen” for Transport Canada. Adopted globally, the Dirty Dozen identifies the twelve most common preconditions that lead to human error in aviation operations. Let’s review the first four. Number one is Lack of Communication: the failure to accurately transmit or receive information, which frequently occurs during rushed shift handovers. Number two is Complacency: a false sense of security that develops during highly routine, repetitive tasks, causing you to miss critical defects. Number three is Lack of Knowledge: performing a task without proper training or relying on memory instead of consulting the current maintenance data. Number four is Distraction: unavoidable or avoidable interruptions that divert your attention from a primary task, causing you to skip critical steps.

Moving on to the next four preconditions. Number five is Lack of Teamwork: a failure to collaborate, share information, or support your colleagues in achieving a safe outcome. Number six is Fatigue: physical or mental exhaustion—often resulting from poor shift scheduling or lack of sleep—that severely impairs reaction time and judgment. Number seven is Lack of Resources: not having the correct tools, replacement parts, personnel, or time to complete a task safely. Number eight is Pressure: the real or perceived demand to meet a strict schedule, which can push you to cut corners to get an aircraft back into service quickly.

And the final four preconditions of the Dirty Dozen. Number nine is Lack of Assertiveness: failing to speak up, ask questions, or challenge a decision when you recognize a clear safety risk. Number ten is Stress: acute or chronic demands—stemming from either the workplace or personal life—that overwhelm your ability to cope and focus. Number eleven is Lack of Awareness: failing to see the big picture or anticipate the future consequences of your immediate actions. And finally, number twelve is Norms: unsafe, unofficial practices or “workarounds” that become the accepted way of doing things, completely overriding official written procedures.

Whether you are physically turning wrenches on the line or managing technical records in the back office, these twelve preconditions constantly exist in your daily working environment. Recognizing the Dirty Dozen, and understanding how they interact within the SHEL model, is the very first step in effective error management.
